WI DHS, Division of Quality Assurance, State-Approved Training Program. This program consists of 75 hours of in-person training (lecture, lab, and clinical experience). The 75-hour requirement includes 8 days of in-person classroom/lab and 2 days of clinicals. Classroom hours are 9am-5pm and Clinicals are from 6:30am-3pm*. Successful completion of the program provides eligibility to take the Wisconsin Nurse Aide Competency Exam administered by D&S Headmaster. *Times are subject to change based on enrollment and/or clinical requirements.

Requirements/Pre-requisites
- Valid State of Wisconsin issued Driver’s License or Identification card or passport. Expired documents are not accepted
- Social Security Card
- At least age 18 or has earned a High School/General Education Diploma. Applicants who are at least age 16 and currently enrolled in high school may be eligible if permitted by the corresponding clinical site for a specific course offering

- Immunizations and/or vaccine immunity titers, a 2-Step TB Test, COVID-19 vaccination or negative test results within 96 hours of patient contact, FLU vaccination (typically required between October 1st – May 31st).
- Be able to meet physical requirements (must be able to be on your feet for an 8-hour clinical shift, lift and transfer 50 lbs, bend, reach, and squat, perform motor skills, have good mobility and balance, and be free of physical and medical restrictions)
- Be able to read and write English at a 9th grade level (minimum)

Required Uniform: black scrub pants, black scrub top, and clean athletic shoes. Backless shoes, clogs, or open-toed shoes are not acceptable.
A completed enrollment form, including required pre-requisites, must be submitted seven (7) days prior to the selected start date.
